Collision Reports

What is it?
The Waterloo Regional Police Service will provide collision reports to:

1. The person or business directly involved;
2. The law firm whose client is directly involved;
3. The insurance company whose insured is directly involved;
4. The person or business who sustained damage to their property as a direct result of a motor vehicle collision.

Information for release?
We will release a photocopy of the collision report.

How much?
$125.00 for each report + HST  = $141.25

How long?
Generally the request can be processed while you wait providing the officer has an opportunity to complete and submit the report to our Records Branch.

Please print the form and complete legibly and accurately. Mail the form with a money order, certified cheque or business cheque (if applicable) in the amount of $141.25 to:

You may also complete the request and pay by cash at the Collision Reporting Centre
